An AMBA Wellness Screening sponsored by DuBois Lions Club PA 14J and hosted by Christ The King Manor will be held on Saturday April 18th from 6:00am until 10:00am.
To schedule your appointment, call 1-800-234-8888. Operators are available Monday through Friday 8:30am to 5:00pm. Please provide your doctor’s name and address when you call to schedule.
Check or money order needs to be made payable to AMBA WELLNESS PROGRAMS.
The DuBois Lions Club also requests a $5.00 donation separate for sponsoring this event. Optional testing is available. Christ the King Manor Community Center is located at 1100 West Long Avenue, DuBois, PA 15801.
DuBois Lions Club Corn Straw Brooms will be available for purchase that day; cost is $15. We hope to see you there!
